Please fill in all of the sections that are relevant to your areas of responsibility. We hope that most of the information is readily available for you, however, we appreciate that some of the financial and cost data may be more difficult for you to identify and provide. If you are unable to provide answers to any of the questions, please be as specific as possible about why not - for example, because your accounting system cannot provide data in this format, or you cannot release the data because of commercial confidentiality.
